Abstract
Background: chronic obstructive pulmonary disease (COPD) is a disease manifested by air current obstruction in airway of breath that is not completely reversible. The patient with this case has repeated inflammation of respiratory tract, hypersensitivity resulting in airway obstruction, and acummulation of secrets. Therefore it is known that the patient has ineffective airway clearance. Objective: to provide an overview of nursing care of fulfilling oxygenation need to Mr. T at Inayah Ward, PKU Muhammadiyah Hospital of Gombong. Discussion: The nursing assessment showed that the patient complain of shortness of breath, difficult to speak, and respiration rate (RR) 26 tpm. The main nursing diagnosis based on data was ineffective airway clearance. The interventions and implementation given to solve this problem were auscultation of breath sounds, assessing dyspnea sign, giving semi-fowler position, giving bronchodilators, providing oxygen therapy, and checking vital signs every 8 hours. Results: Evaluation done for three days showed that the patient said his shortness of breath still exist, RR 22 tpm, patient can do an effective cough tehcnique, the problem has been soved partly. Therefore it was needed to give the patient nebulizer with additional magnesium sulfate to improve pulmonal tissue and minimal side effect.
