Abstract

Background: Minimum service standard of hospital according to government regulation states that the incidence of medication errors must be 0% in health services. The incidence of medication errors at “x” hospital was 28% of the total incidences report in 2020. Health professionals have important role at preventing medication error. Objective: To find out the incidence of medication error at “x” hospital. Method: Qualitative research design with a case study approach. The participants of this study were health professionals who were involved in patient safety incidence at ‘x” hospital of 8 participants taken by purposive sampling technique. Data were collected by in-depth interview and study of the documentation of patient safety incidence report. Analysis of the data used 7 steps according to Colaizzi assisted by the OpenCode application version 4. Result: there were several themes including perceptions of health professionals about medication errors, the role of medication administration, factors causing medication error, the result of medication error, and the response of health professionals after medication error. Conclusion: The incidence of medication error at “x” hospital is not only triggered by health professionals, but also environmental and medication factors.
