Abstract

Background: The inflammatory process of bronchopneumonia causes the production of secretions increases causing several symptoms resulting in ineffective airway clearance as a nursing problem. Constraints that often occur in preschool children are the inability to expel phlegm because of preschool age the cough reflex is still weak and secretions are swallowed more often. One of the actions that can be taken to overcome the problem of ineffective airway clearance is by applying PLB therapy. The pursed lips breathing intervention is to increase the alveolar pressure in each lung lobe so that it can increase airflow during expiration. Increased airflow during expiration will activate the cilia on the airway mucosa so that it is able to evacuate secretions out of the airways. Objective: to explain nursing care of broncopneumonia patients with ineffective airway clearance as the main nursing problem given Pursed Lips Breathing (PLB) therapy at Prof. Dr. Margono Soekarjo hospital. Methods: The research design is descriptive with case study approach of five bronchopneumonia patients with the main nursing problem of ineffective airway clearance given PLB therapy. Results: The results of the analysis of nursing treatment innovations (before and after) of the five patients managed with PLB therapy showed that they experienced improvement based on the decrease of respiration rate (RR), more productive sputum, and additional breath sounds were faded. Recommendation: The results of this study can provide public knowledge on how to treat children when coughing is ineffective and unable to produce sputum by giving PLB therapy independently.
