Abstract
Background: Data obtained from (Riskesdas) in Indonesia, especially Yogyakarta in 2013, the prevalence of iron deficiency anemia nationally in adolescents aged 13-18 years at 22.7%. Objective: to describe nursing care on Mr. S in Dahlia Ward, Dr. Sudirman Hospital Kebumen. Discussion: The nursing diagnoses were ineffective peripheral tissue perfusion associated with monotonous lifestyle, activity intolerance related to general weakness, deficiency of knowledge associated with less exposure. Intervention and implementation conducted by the authors on nursing problems which give antibiotic therapy, give a blood transfusion PRC, checking vital signs, Observing their activity limitation, observe their activity limitation, train clients to eat their own, helping the client to the bathroom, assess the client's knowledge and family about the illness client. Results: the evaluations of nursing diagnoses three day one diagnosis unresolved is ineffective peripheral tissue perfusion associated with monotonous lifestyle.
