Abstract

Background: Congestive Heart Failure (CHF), particularly among the elderly, is associated with high prevalence, mortality, and readmission rates. Reduced cardiac output exacerbates symptoms such as dyspnea. Alongside pharmacological therapy, Deep Breathing Exercises (DBE) have been shown to effectively improve respiratory function and efficiency, while also reducing dyspnea in patients with CHF. Objective: To analyze nursing care for patients with congestive heart failure and reduced cardiac output at PKU Muhammadiyah Petanahan Hospital. Methods: A descriptive method with a case study approach was employed in this study. The research involved five patients diagnosed with CHF and reduced cardiac output. Instruments used included a nursing care format, a Nursing Kit, and the Standard Operating Procedure (SOP) for the innovative Deep Breathing Exercise intervention. Data were analyzed qualitatively and presented in both tabular and narrative formats. Results: The assessment revealed that all five patients reported the primary complaint of shortness of breath. Data analysis indicated consistent symptoms among the patients, such as increased shortness of breath during activity, fatigue, and occasional coughing. These findings led to a nursing diagnosis of reduced cardiac output related to changes in heart rhythm, preload, and afterload. The planned nursing interventions were comprehensive, including monitoring vital signs and symptoms, adjusting patient positioning, providing relaxation therapy, offering emotional and spiritual support, administering oxygen, recommending gradual activity progression, educating on fluid balance, collaborating on antiarrhythmic medication, and implementing cardiac rehabilitation programs that included Deep Breathing Exercises. The intervention's implementation focused on DBE, and evaluation showed significant improvements, including reduced shortness of breath, respiratory rate, and tachycardia, as well as increased oxygen saturation. The use of DBE was identified as an innovative and effective nursing intervention for CHF patients with reduced cardiac output. Conclusion: The implementation of Deep Breathing Exercises effectively alleviates symptoms in CHF patients with reduced cardiac output, particularly by reducing shortness of breath, respiratory rate, and tachycardia, and improving oxygen saturation. Recommendation: Deep Breathing Exercises should be integrated into standard nursing care for CHF patients with reduced cardiac output to enhance patient outcomes.
