Abstract
ASSESSMENT : Name Mr. S, aged 40 years, male gender, address Cilacap, religion Islam, elementary education, private employment, ethnic Javanese. Patients come to the IGD RS PKU Muhammadiyah gombong on 28-05-2016 at 13:00 pm with fever already four days before entering the hospital. Clients say dizziness, nausea, but not vomiting, limp, decreased appetite, mouth tastes bitter. At the time in the IGD examination vital signs with the results of the temperature: 38.50 C, blood pressure: 110/80 mmHg, pulse: 78x / minute, respiratory rate: 20 times / min, do infusion RL 20 tpm. NURSING DIAGNOSIS : Nursing diagnoses that arise are nausea associated with increased gastric acid, deficiency of knowledge related to the lack of resources, the risk of bleeding associated with the inherent coagulation (thrombocytopenia). INTERVENTION : A plan of action has been made that diagnosis 1. examine the level of knowledge of the client and family. 2. provide health education appropriate level of understanding of the client. 3. provide client motivation to learn. IMPLEMENTATION : The action has been carried out, namely: 1. Assess the level of knowledge of the client and family. 2. Provide health education appropriate level of understanding of the client. 3. Provide the client's motivation to learn. EVALUATION : Evaluation of three nursing diagnoses that appear in nursing care has been resolved two diagnoses are nausea and knowledge deficiency.
