Methods: This research used an analytic correlation method with cross sectional approach. There were 83 respondents meeting the inclusion criteria selected by purposive sampling technique. The collected data were then analyzed using univariate analysis, a Chi Square test, and regressive-logistic as multivariate test. Results: This research found that 73 patients (88,0%) experienced a preopertive anxiety. The Chi Square test showed some factors connstributing to anxiety level, such as a gender with p value = 0,016 (p < 0,05) and family support with p value = 0,011 (p < 0,05). The regressive-logistic multivariate test showed OR value of family support was highest, wich was 2,719. Conclusion: The most dominant factor affecting anxiety level of preoperative patients in PKU Muhammadiyah Gombong Hospital was family support.
